Insurances we accept
Below is a list of insurance providers we accept. We also accept self-pay, cash-pay patients, HSA and flexible speding accounts as well as Care Credit. Please contact our office to confirm coverage prior to your appointment.
Eye Focus Northwest accepts a wide range of insurance plans and will bill insurance as a courtesy. However, the primary responsibility for your account remains with you. Please bring your current medical and vision cards to your appointment.
Some insurance plans may have restrictions or a designated provider network that you must use. Be sure to verify with your insurance provider regarding your eligibility, benefits, provider network, and any referral requirements that may apply to you.
